MINUTES OF PUBLIC MEETING

FOR CORRECTIONS OFFICER RETIREMENT PLAN LOCAL BOARD

ARIZONA DEPARTMENT OF CORRECTIONS

A Public meeting of the Corrections Officer Retirement Plan Local Board for the Department of Corrections was convened, Thursday, January 22, 2004, at 9:00 a.m., in the Occupational Licensing Building, 1645 West Jefferson, Fourth Floor Conference Room, Phoenix, Arizona.

Present at the meeting were the following individuals:

Nolan Thompson, Chair Lu Mc Lendon, Board Secretary

Patrick Scherden, Board Member Karen Ray, Board Coordinator

Mike Smarik, Board Member Carol Osterhaus, PSB

I. Call to Order -The meeting of the Corrections Officer Retirement Plan Local Board was called to order by Chair, N. Thompson at 9:24 a.m. with board member, M. Smarik attending via telephone - Election of Local Board Chair for 2004 was tabled for the next regularly scheduled board meeting in February 2004.

II. Approval of Minutes - Approval of the December 23, 2003, Public Meeting Minutes - Tabled.

III. Disability Retirement - The Local Board may consider and vote on the following disability applications. The Board may defer decision to a later date. The Local Board my vote to discuss these applications in Executive Session pursuant to A.R.S 38-431.03 (A)(2)

a. 99-03 - Re-evaluation - Tabled

b. 98-06 - Re-evaluation - Tabled

IV. Normal Retirement - The Local Board may consider and vote on the following applications for Normal Retirement in public meeting or defer decision to a later date. P. Scherden made a motion to approve the Normal Retirement application for Paul F. Brooke, Jr. pending receipt of a signed C12 and further motioned to approve applications b - k. Seconded by M. Smarik. Approved

V. Survivor Benefit - The Local Board may consider and vote on the following applications for Survivor Benefits in public meeting or defer decision to a later date.

a. Alicia V. Hines (Tony) - A motion for approval was made by P. Scherden. Seconded by M. Smarik.

VI. Death Benefit - The Local Board may consider for the record the following applications for Death Benefits in public meeting or defer decision to a later date.

a. Barbara Tucker (Antonetta Campers & Takisha Morgan) -Noted.

VII. Request to Remain in ASRS/CORP - The Local Board may consider and vote on the following waiver requests in Public Meeting or defer decision to a later date.

a. Victoria Granillo (CORP)

a. Simon N. Obi (ASRS)

a. Gregory G. Yardley (ASRS)

VIII. General Discussion

a.  Job Class eligible for CORP - DC Correctional Education Program Spect. (#04833) uncovered. Tabled

A motion to adjourn was made by P. Scherden. Seconded by M. Smarik.
