Inmate Visitation Application

Effective immediately, family and friends may complete and submit the Application to Visit an Inmate, Form 911-4 .

There are two links:

1. One link is for potential visitors who would like to submit their application electronically.
2. The second link is for potential visitors who do not wish to send the application electronically. The application will need to be printed, completed and mailed to the institution where the inmate is housed.

In both instances it is critical that all information fields are completed prior to submitting. The electronic submission of visitation applications is only available at this time for family and friends of inmates who are assigned to State institutions. If an inmate is assigned to a Contract Bed in Arizona or Oklahoma, potential visitors must print and complete their application and mail it to the institution where the inmate resides or the inmate may mail an application to the potential visitor to complete before it is mailed back to the institution. If the potential visitor does not have access to the Internet, the inmate must mail the application regardless of the location.
