CORRECTIONS OFFICER RETIREMENT PLAN LOCAL BOARD
FOR ARIZONA DEPARTMENT OF CORRECTIONS

AGENDA

 

The Meeting of the Local CORP Board for the Arizona Department of Corrections (ADC) will be held in the 2nd Floor Computer room of the ADC office at, 1831 W. Jefferson, Phoenix, Arizona, 85007, commencing at 10:00 a.m. on   May 2, 2013. The Board may vote to hold an executive session, which will not be open to the public, to discuss certain matters.  The Board reserves the right to consider agenda items out of their listed order.

I.       CALL TO ORDER, ROLL CALL, OPENING REMARKS -

II.      APPROVAL OF MINUTES -

      a. Approval of Public Session Minutes from April 4, 2013 meeting.

III.      DISABILITY RETIREMENTS-Items may be discussed/voted on by the Board. At any time, the Board may enter into Executive Session pursuant to A.R.S 38-431.03 (A) (2) and obtain legal advice pursuant to A.R.S 38-431.03 (A) (3) to discuss details of a serious or confidential nature and A.R.S. 38-841.03 (2) - discussion of records exempt by law from public inspection (e.g. medical records). The Executive Session may be required to discuss medical histories.

      a. Keri Blasé - Ordinary Disability - Set up IME on May 10, 2013.  Appointment was made at the Tucson office.
      b. Paul Dumm 13-05 - Ordinary Disability – Medical documentation sent to Board for review to determine if an independent medical exam.
      c. Humberto Garcia - Ordinary Disability – Appeal denial of Ordinary Disability. According to PSPRS he refunded his money on 6/15/2012 and is not qualified for a disability. Sent certified letter on April 29, 2013.
      d. Paul Gillum- Ordinary Disability - Disability was denied. Wrote a letter to the governor requesting assistance. Michael Smarik wrote a reply.
      e. Daniel Hernandez - Approved for ordinary disability, request benefits to start on March 31, 2013.
      f. Renee McDuffie - Ordinary Disability - Reapplied for disability. Member was informed of the definitions of the three types of disabilities. Submitted appeal and changed to ordinary disability.

IV.       NORMAL RETIREMENTS - The following applications have been filed and are presented for Local board consideration:

V.      SURVIVOR BENEFITS – Items may be discussed/voted on by the Board. At any time, the Board may enter into Executive Session pursuant to A.R.S 38-431.03 (A) (2) and obtain legal advice pursuant to A.R.S 38-431.03 (A) (3) to discuss details of a serious or confidential nature. The following applications have been filed and are presented for Local Board consideration:

      a. Frances Griffiths (Charlotte)

      b. Robert Penn (Yasuko)

VI.      DEATH BENEFITS – Items may be discussed/voted on by the Board. At any time, the Board may enter into Executive Session pursuant to A.R.S 38-431.03 (A) (2) and obtain legal advice pursuant to A.R.S 38-431.03 (A) (3) to discuss details of a serious or confidential nature. The following applications have been filed and are presented for Local Board consideration:

      a. Ryan Purdy (Kathleen Johnson)

VII.      REQUEST TO REMAIN IN ASRS/CORP – The following applications have been filed and are presented for Local Board consideration:

      a. None.

VIII.      GENERAL DISCUSSIONS – Items may be discussed/voted on by the Board. At any time, the Board may enter into Executive Session pursuant to A.R.S 38-431.03 (A) (2) and obtain legal advice pursuant to A.R.S 38-431.03 (A) (3) to discuss details of a serious or confidential nature.

      a. Robert Duffy - Applied for Retiree Accumulated Sick Leave, was denied based on RASL requirement that last day worked and first day of retirement must be within 31 days. Letter from attorney requests the Board change his retirement date.

      b. Communications Director/AUN04843/27 – New position created. Oversee media and public relation function. Management recommends approval under ASRS.

IX.       CALL TO PUBLIC - This is the time for the public to comment. Members of the Local CORP Board may not discuss items that are not specifically identified on the agenda, except to address criticism from the public. Therefore, pursuant to A.R.S. § 38-431.01(H), the Local CORP Board’s reaction to any public comment is limited to addressing criticism or recommending that the Local CORP Board or Staff responds or study such comment or schedule the subject matter for further consideration at a later date after appropriate notice.
