Director’s Instruction # 256, Transfer of Medical Orders between Health Facilities

This Director's Instruction is effective immediately and will remain in effect until incorporated into the appropriate Department Order.

Purpose:

This document delineates the procedure for the transfer of inmates between facilities specific to the continuity of care and of health care information.

Procedure:

Effective with this Instruction, inmates’ treatment records and health and mental health care information will be transferred to the receiving complex health services facility within the timelines provided.

1.0    The standard of care is to notify the receiving facility that a medical prescription order has not been filled for an inmate prior to their transfer. Transferring facilities will ensure that all ordered care is properly documented.

2.0    Documentation of care and pending care must be entered into the medical record prior to transfer.

2.1     If the inmate is transferred prior to completion of the pending medication treatment plan, the progress note shall be faxed the same working day of the inmate’s transfer to the receiving complex’s health facility.

2.2     The Medical Continuous Progress Record (Form #1101-62P) containing the medication treatment plan that requires clinical and/or pharmacy action, will be faxed. A receipt shall be printed and a call shall be made verifying receipt of the Record sheets. The sending of the document shall then be recorded by nursing entry in the Progress Record with the name of the Medical/Nursing individual sending the form, the identification of what was sent to where, and the name of the individual acknowledging receipt.

2.3     The Mental Health Continuous Progress Record (Form #1103-40P) containing the medication treatment plan that requires clinical and/or pharmacy action, will be faxed. A receipt shall be printed and a call shall be made verifying receipt of the Record sheets. The sending of the document shall then be recorded by Mental Health staff entry in the Mental Health Continuous Progress Record with the name of the Mental Health individual sending the form, the identification of what was sent to where, and the name of the individual acknowledging receipt.

2.4     The Medical Continuous Progress Record and Mental Health Continuous Progress Record forms shall be forwarded to the receiving facility for review by the unit medical and/or mental health provider. Following review by the provider, the original Continuous Progress Record and the faxed copy shall be immediately filed chronologically in the patient’s Medical Record in accordance with Department Order 1104 and the Health Services Technical Manual.

3.0     Documentation of prescription medication and pending provision of prescription medication must be entered into the medical record prior to transfer.

3.1     If the inmate is transferred prior to filling or delivery of the ordered medication, the prescription shall be faxed the same working day of the inmate’s transfer to the receiving complex’s supporting pharmacy.

3.2     The Prescription Record (Blue) (Form #1101-81P) describing the medication that has not been filled or delivered, will be faxed by the sending Pharmacy. A documented receipt shall be printed and a call shall be made to the receiving Pharmacy verifying receipt of the Prescription Record. The original form shall be logged immediately in accordance with current Arizona Pharmacy Regulations and Health Services Technical Manual guidance. The recording shall include the name of the Pharmacy individual sending the form and the name of the Pharmacy individual acknowledging receipt.

3.3     The original forms shall be forwarded to the supporting Pharmacy for review by the Pharmacist and filing in Pharmacy Records in accordance with Board of Pharmacy rules and Health Services Technical Manual.
