  1. How do I request to be notified upon an inmate’s release from the Arizona Department of Corrections custody?

    A victim request for notice must be sent to the Arizona Department of Corrections, Office of Victim Services, 1645 W. Jefferson, Phoenix, Arizona 85007, (602) 542-1853 utilizing the pink copy of the Post-Conviction Notification Request form. If you, as the lawful victim/representative, do not have a form available, you may request one by writing or calling the Department’s Victim Services. If an inmate is returned to custody in the Arizona Department of Corrections for a violation of release conditions and/or return from escape/abscond status, the Release Notification Unit will notify a victim of the inmate’s return.
  2. How do I request not to receive inmate mail?

    Pursuant to ARS 13-4411.01, which became effective January 29, 2000, victims may request NOT to receive inmate mail and the Department shall not knowingly forward mail addressed to any person who requests not to receive mail pursuant to this section. The victim’s request must be indicated on a Post-Conviction request form and submitted to the Office of Victim Services. The Department of Corrections will send a revised post-conviction notice to any victim who would like to make this request.
  3. How do I receive notification of parole or clemency hearings?

    The Arizona Board of Executive Clemency will send notification for any upcoming parole or clemency hearing. It is important that you keep your address current with the Board of Executive Clemency in order to be kept informed. Victims have the right to attend the hearings or provide input in writing. It is important that you contact the Board of Executive Clemency for the specific date, time and location of the hearing.
